Your child's Health History and Release of Liability form will be sent to you via email or postal mail after you register. We must have this form for all campers to attend camp

Permission to Administer Medication Form  -If your child will take medication while at camp.

Unimmunized Child and Refusal of Treatment Waiver  -If your child is not immunized or we should not treat them in the case of a medical emergency.

 Travel Camper Health Recommendations - Filled Out By Doctor -For all campers in River Trek & Backpack Adventure, Wilderness Challenge and Teen Backcountry Adventure: North Manitou

ACPL Student Medical Form  -For all campers in Leadership in Nature
